PRIVACY ACT/FAIR TRADING ACT PROVISIONS (to be read by applicants)

* Information on this site is collected by Videohub and is used solely by Videohub for the purpose of keeping track of and/or recovery of goods hired from the Videohub.
This is an application for membership for the purpose of hiring goods from the Videohub and for credit for the value of outstanding goods hire and/or for charges relating to overdue and hired goods.
* I consent to the above information being passed to, and to receive reports from our appointed agents, for the purpose of evaluating credit worthiness, and also with relevant hiring details for the purpose of recovering outstanding goods or fees.
* I acknowledge that if hired goods are not returned within the time period, and Videohub has to facilitate the return of the hired goods then the privacy of my information as recorded by the Videohub may be compromised.
* Where a library member has not returned hired goods within 5 days from the expiration of the hire period and Videohub has attempted to contact the member by telephone or by letter sent to the last known these address. The goods are deemed to be converted by the member. Conversion is a form of theft and is a criminal offence.
* If recovery steps have to be taken by Videohub or the Videohub agent I will be liable for any associated legal and recovery costs.
* I acknowledge that where listed goods have been lost or damaged by me I am liable for the charge for such loss or damage (refer clause 2.6 in on the Terms of Agreement.)
* The member has the right upon request to access to information stored on that member by Videohub and to correct such information if- necessary
* If the applicant fails to supply or incorrectly supplies the information requested, Videohub reserves the right to refuse or cancel membership
* I acknowledge that if I allow other people to use my membership hiring purposes then the privacy of my information as recorded by the Videohub may be compromised.
* I am 18 years of age or older. I acknowledge that the terms and conditions contained on the back of this form have been pointed out to me and that I have either read these terms or had the opportunity to read them before signing or declined to read the terms.

* You can request amendments or deletions to our database by the Contact Us page or visiting the shop.
* Any areas of the web site that allow public submission of information may also gather your current IP address. This information may be used to track and control abuse.
